Include your Shopify order number, purchaser email, CareID product, and one of these request types:
- Personalized setup instructions
- Profile update request
- Resend digital materials
- Lost, damaged, or replacement CareID
Do not place medical details in this form. Do not submit medication lists, allergies, diagnoses, dates of birth, addresses, caregiver notes, or other sensitive information through a general support message. CareID will provide the appropriate intake or update instructions after identifying the order.
